After Stroke partners with a growing number of hospitals, rehabilitation centres, healthcare professionals and community organizations across Canada.

These partnerships allow us to reach stroke survivors and their families with personalized resources and services throughout their recovery journey.

Working with Heart & Stroke Foundation

Our collaboration with Heart & Stroke builds on our shared vision for stroke services in Canada.

Announced in May 2021 , and anchored in the lived experiences of stroke survivors and their families, our enhanced program draws on the complementary strengths, expertise and resources of both organizations to help optimize recovery.

The partnership focuses on:

  • Integrating stroke best practices into community-based services by sharing the most recent evidence-based guidelines, resources, and materials for professional and public audiences.
  • Working together to build on-the-ground capacity for high-quality stroke support, including education and training programs.
  • Creating new resources and fresh learning opportunities for people living with stroke and their families.
